چکیده

The ideal -method for the treatment of acromegaly remains uncertain. Evidence of local pressure from a pituitary tumour offers an absolute indication for treatment, and this has usually been surgical decompression ; however, when treatment has been given by external deep x-ray therapy the majority of such patients have shown improvement in their visual-field defects (Fraser and Joplin, 1961). Where the presenting syndrome comprises only acromegaly and the other effects of excessive growth-hormone secretion the need for treatment may seem less obvious. The activity of acromegaly is difficult to assess, and the natural history of the untreated disease is not adequately known. Bishop and Briggs (1958) reported that 80% of their patients had died by the age of 60 predominantly from cardiovascular complications, but this high mortality rate awaits confirmation. Acromegaly without symptoms of a pituitary tumour has often been left untreated; however, we have implanted such cases as well as those with tumour signs

Partial pituitary ablation with implants of gold-198 and yttrium-90 for Cushing's syndrome with associated adrenal hyperplasia.

There is still considerable controversy over the best method of treating Cushing's syndrome when it is due to bilateral adrenal hyperplasia. The majority of such patients are treated by either total or subtotal adrenalectomy. These, however, are not only major operations but they often lead to a lifelong dependence on corticosteroids, and are sometimes followed by progression of a pituitary tum...

Treatment of Nelson's syndrome by pituitary implantation of yttrium-90 or gold-198.

Eight patients with Nelson's syndrome were treated with a pituitary implant of yttrium-90 or gold-198 four to 16 years after adrenal surgery. All had considerable pigmentation. One already had cranial nerve abnormalities and visual field defects and had had both a craniotomy and deep x-ray treatment. Radiographs showed that the pituitary fossa was abnormal in seven patients. A biopsy performed ...

Repeat pituitary ablation for advanced prostatic cancer.

An elderly male patient with advanced prostatic adenocarcinoma, requiring increasing analgesia, underwent interstitial pituitary irradiation by implantation of yttrium-90. He was temporarily relieved of his pain. Assessment of his anterior pituitary function showed only partial ablation. He therefore underwent a second implantation. This achieved complete ablation, and only when this was achiev...

Serum growth hormone levels and the response of diabetic retinopathy to pituitary ablation.

Serum growth hormone levels were measured during insulin tolerance tests in 36 patients after yttrium-90 pituitary implantation for diabetic retinopathy. The response of the new blood vessels was more clearly related to loss of growth hormone function than was the improvement of retinal haemorrhages and microaneurysms. The overall response of the retinopathy was greatest when growth hormone fun...

Long term effect of external pituitary irradiation on IGF1 levels in patients with acromegaly free of adjunctive treatment.

OBJECTIVE It is established that external pituitary irradiation (EPI) effectively reduces serum GH levels in acromegaly. However, its effect in normalising serum IGF1 has been disputed. We looked at the number of our patients who achieved persistently normal IGF1 levels whilst free of adjunctive treatment for at least 1 year after EPI. PATIENTS AND DESIGN We identified 63 acromegalic patients...
